補課通知

付款前安排

• 學員如在付款前，已得悉該課程中將會缺席的日期及堂數，可於報名表格填寫將缺席的日期，並可每課 $200 (PeeWee則$100)扣除將缺席的堂數，即如需缺席2課，則可扣除$400 (PeeWee則$200)。為確保學員學習進度, 如需在繳費前申請缺席費用減免，只能允許最多減免2堂費用。

付款後安排

• 本學院不會提供任何補課安排，只會視作缺席論，並不會退還缺席款項，敬請留意。

病假通知

• 如學員身體不適以致未能上課，並能提交醫生証明書，學員可於下期課程扣除每課  $150 (1.5-2小時課程) / $75 (1小時PeeWee課程) 。

報名表格內所填報的資料必須全部屬實，並須詳細填寫各項資料，報名表格一經遞交，直至課程完結前，所有填報資料不得更改(通訊電話除外)。

未滿十八歲的申請人必須由其家長或監護人簽署聲明書後，才可參加課程。

參加者須把填妥的申請表格連同「聲明書」及寫有「GMF Limited」的劃線支票，寄回以下地址﹕

本學院保留拒絕任何參加者參加訓練的權利。

Make Up Lesson Arrangement

Before Submission

• If students inform GMF the absent date BEFORE submission of the Enrollment form and Program Fee, $200 per lesson (PeeWee $100/lesson) is allowed to be reduced in the program fee. To maintain the players basketball development, Max 2 lesson fee is allowed to deduct if need to apply absent BEFORE the payment is made.

After Submission

• Make up lesson is no longer be provided for any reasons, GMF will treat it as absent only and no refund will be arranged.

Sick Leave

• If a student is sick with the provision of medical certificate, GMF allows reducing$150 per lesson (For 1.5-2hr lesson) / $75 per lesson (For 1hr PeeWee lesson) for the application in the forthcoming program。

 

Please insure information provided on the enrolment form is accurate and completed in full.  Should your contact number or occupation change prior to the end of the Program, please inform us in order to keep our records straight.

Applicants below the age of 18 will be asked to complete a declaration completed and signed by their parents. All applicants will be asked to sign an injury waiver.   Although it is the intention of the GMF and its staff to shield all participants from injury, we must be aware that in competitive setting injuries may occur.

All applicants should return the completed enrolment, parent declaration and accident waiver forms along with a cheque payable to:

The GMF reserves the right to disallow anyone from taking part in training program.

Inclement Weather Arrangements

如於課堂開始前3小時，遇以下惡劣天氣情況，課堂將予取消﹕
1. 黑色暴雨信號；或
2. 8號預警（即天文台在確定發出8號信號前的兩小時內發出之預警信息）；或
3. 8號或以上熱帶氣旋警告。

惟如上述暴雨信號或熱帶氣旋警告於課堂開始前3個小時已除下，課堂將照常進行。

Should Pre-No.8 / Black Rainstorm Signal special announcement (an advance notice to the public issued by the Hong Kong Observatory when the tropical cyclone warning signal No. 8 / Black Rainstorm is expected within two hours), tropical cyclone warning signal No. 8 or above or black rainstorm signal is in force 3 hours before the lesson, all lessons will be cancelled. 

GMF lesson will resume normal when the tropical cyclone warning signal No. 8 or above or black rainstorm signal were lowered 3 hours before the lesson starts.
